Please complete the  Student Directory Release Form IF you would like contact information (address, phone number and parent names) withheld on the PowerSchool Student Directory OR IF you would like ALL information regarding your daughter withheld on all publications - yearbook, student directory, transcript services, newspaper, etc...

We will assume consent if the form is not returned and your daughter's content information will display in the directory, yearbook, newspaper.
  

If you print and complete the form for information to be withheld, please have your daughter return it to the Reception Area by Thursday, September 1, 2016.
